Part I Paperwork due February 1st:

• Personal Information Form
• Consent, Release, and Participation Agreement
• Passport Information Form (must have valid passport ready)  
• Confidential Physical Health Form
• Confidential Mental & Behavioral Health Form (applicable only to volunteers who are seeing or have, in the past two years, seen a treating mental health clinician)
• Immunizations Record - submitted with Physical Health Form. See immunizations list here!

 Part II Paperwork due April 1st:  

• Consent for Travel of Minor & Power of Attorney - only applicable for volunteers under 18 at time of project start
• Medical Power of Attorney - only applicable for volunters 18 years or over at time of project start
• Health Update
• Background Check & Criminal Record Affidavit
• Rooming Preference Form
• Volunteer & Family Questionnaire - parents should complete this for volunteers under 18; volunteers 18+ can complete this for themselves) 
• In-Country Release & Travel Deviation Request and Agreement - only applicable for volunteers with flight patterns different than the group or flying out of and into different airport
